Please bring a copy of your photo ID and all insurance cards

  • check with your medical insurance if referrals are needed to see the physicians
  • if referrals are needed, you will need one referral for the physician and one for the treatment facility
If you have a pacemaker/defibrillator, please bring copy of Pacemaker/Defibrillator Card
If you had previous radiation treatments, please bring facility name/number with you to obtain any treatment related records
Please complete new patient questionnaire and medication list (our nursing staff will assist you if you have any questions or trouble)
